CTLTC Membership and Payment Form

Please fill out this form to complete your CTLTC Membership.

Please note that

1. Country members must be based at least 30km or more from the club.

2. If you are registering two adult members only, with no junior members or family members under 26 years of age in full-time education, it is recommended that you select Dual Membership rather than Family Membership.

Membership Declaration:

By submitting this membership information, you are agreeing to each of the following: 

1. Payment of Fees: I/We would like to join CTLTC and will pay the appropriate membership fees. 

2. Club Rules: I/We agree to be governed by the rules and regulations of the Club as outlined at https://www.tltc.ie/club-rules-and-policies for the time being in force and any which may hereafter be issued.

3. Club Assistance: I/We agree that membership will entail provision of such assistance at the Club as may be required by the Club Committee from time to time. 

4. Data Protection Statement: I/We agree that the club can store the details provided in this form and that this data can be used by the club for club administration and communications with me/us via text messaging, phone calls and email and that this data can also be shared with third-party applications used by the club for club and court administration, booking and payment systems.

5. Code of Conduct for Parents and Children: By completing this Membership Application Form, I/We confirm that I/We agree to abide by the Code of Conduct for Parents and the Code of Conduct for Children as outlined on the CTLTC website at https://www.tltc.ie/club-rules-and-policies 

6. Disclaimer: I/We agree that CTLTC, its Committee Members/Officers or Trustees cannot accept any responsibility or liability for damage, accidents or injury to either property or individuals incurred on Club premises or grounds. All persons on-site are required to comply with the rules of the Club and adhere to safe practices at all times when on Club property. Children (under 18-year-olds) cannot be left unsupervised at any time on Club premises or grounds and parents/guardians are vicariously liable for the behavior of their children. 

7. Photography and social media/web: I/We consent to having my/our and/or our child's/children's photos from tennis events displayed on the club’s website, social media page, and in club promotional literature. However, if you do not wish to have you and/or one of your childs photos taken, all reasonable effort will be made to accommodate such requests. 

8. Membership Approval: All new membership applications must be reviewed by the Club Committee prior to acceptance.
